NEW CUSTOMERS:
A brief Trade Information application must be completed and approved prior to order processing. For your convenience, the Trade Information application is provided in a PDF format above. Please print the page, complete the information requested, and fax or mail it to us.
PAYMENT:
Payment is expected at the time of order, or must be received prior to order processing. If trade references have been submitted and approved, payment must be remitted net 30 days from the date of invoice. Accounts past 45 days will be placed on hold. Mastercard, Visa and Debit cards are accepted. The fee for returned checks is the maximum allowed by law.
EXTRA SIZE AND DESIGN CHARGES:
Add 15% for XX Large; 35% for XXX Large; 75% for XXXX Large. Design Charge: $25.00 design setup charge.
RETURN POLICY:
No returns accepted for custom made products. No returns for any reason will be accepted without office authorization and assignment of return number. Returns made within 45 days of receipt of merchandise will be subject to a 15% restocking charge. Returns made 46-90 days after receipt of merchandise will be subject to a 25% restocking charge. No returns will be accepted after 90 days. All orders are confirmed with the customer to be sure the correct products are manufactured. Due to hygienic reasons, medical product items may not be returned after use.
WARRANTY:
Our employees work hard to produce orthopedic products of the highest standard. Each product is inspected before leaving our facility to be sure we only ship the BEST quality, however, if an item does not meet your satisfaction, please contact us immediately and we will work with you to find a reasonable resolution.
SHIPPING:
All shipping charges are the responsibility of the customer and are added to the invoice. We utilize UPS or FedEx services for all shipments. Shipping rates to locations outside the continental U.S. are available upon request. The following rates apply to the Continental United States. Weekday early AM and Saturday deliveries are available at an additional cost. Freight: FOB Hickory, NC. Freight prepaid and added to invoice. There is a $5.00 charge for each drop shipment. There is a $20.00 handling fee for Export orders. A $2.50 handling fee applies per shipping carton.

PACKAGING:
Products are packaged in white boxes or sealed poly bags, please state your preference when ordering. All products are labeled with the appropriate HCPCS code for medical claims submission.
